8 Steps to Serving Your Guardianship Documents

1.  Mail copies “Petition for Guardianship” and “Notice of Hearing” forms 1                               days before the court hearing.


2.  Take your certified, or stamped copies, to the
                            post office.


3.  Mail your copies to each person in your case. 
                            Mail the copies “certificate of mailing” with a “return receipt”.
                            Ask the post office for receipts for each mailing.  Keep the receipts.


4.  Print 2 forms “Statement of Mailing” and “Exhibits 1&2"
                            from this website here. You can also pick up the forms
          from the Circuit Court.


5.  Fill out the forms on a computer or with a blue or black pen. 
          Sign the forms.


6.  Make 4 copies of the forms for the court.  
          Keep 1 copy for your files.


7.  Drop off the original form and copies with the clerk on the 1st Floor
          of the Circuit Court.               


8.  The clerk files your original and copies. 
          Filing means the clerk stamps the forms. 
                            The clerk will keep the original and one copy. 
                            Keep the extra stamped copies for your files.
